Tutorial: View Health Details
Use this tutorial to view data on the child's health information on the Health page.
Complete the following steps to perform this tutorial:

Display the Health page.

Click the [+] to expand the Physician Health Summary section. The section expands to display additional information.

View the physician health summary details.
The Physician Health Summary section contains the health summary data:
Received From displays the name of the individual from whom the data was received.
Date Received displays the date on which the data was received.

Double-click on a physician in the Physician table. The page refreshes and displays detailed information below the table.

View the physician details.
The Physician section contains the physician details data:
Name displays the person's first and last name.
Address displays the street address, city, two-letter state abbreviation, and zip code.
Phone displays the telephone numbers at which the person (or primary contact) can be contacted.
Hospital displays the name of the hospital with which the physician is affiliated.
Physician Type displays the specific role assigned to the physician from the drop-down list.
Start Date displays the date on which the person (or group) becomes (or became) active or assigned.
End Date displays the date on which the person (or group) becomes (or became) inactive or unassigned.
Date Last Seen displays the date on which the person was last seen by the physician.
Number of Visits displays the number of times the person has been seen by the physician.
Reason for Visit displays the purpose of the visit.

Click the [+] to expand the Neonatal section. The section expands to display additional information.

View the neonatal details.
The Neonatal section contains the neonatal data:
Date of Birth (DOB) displays the month, day, and year on which the person was born.
Actual Due Date displays the month, day, and year on which the person is or was expected to be born.
Gestational Age (Weeks) displays the person's gestational age in weeks.
Birth Weight (Grams) displays the person's birth weight in grams.
Birth Hospital displays the name of the hospital at which the person was born.
Adopted check box indicates that the person was placed for adoption.
Foster Child check box indicates that the person was placed in foster care.
The APGAR score 6 or less at 5 minutes check box indicates that the child had an APGAR score of six or less, five minutes after birth. Then, view the APGAR score.
The Intracranial Bleed check box indicates that the child had an intracranial bleed at birth. Then, view the grade.
The Vent/CPAP Dependent check box indicates that the person was dependent on a ventilator or CPAP after birth. Then, view the number of days.
The Asphyxiation check box indicates that the child asphyxiated.

The General Health section contains the general health data:
Concerns displays any comments or concerns regarding the child's general health.

Click the [+] to expand the Diagnosis section. The section expands to display additional information.

View the diagnosis details.
The Diagnosis section contains the diagnosis data:
ICD9 Code displays the ICD9 code of the diagnosis.
Category 1 identifies the category 1 code of the diagnosis.
Start Date displays the date on which the person (or group) becomes (or became) active or assigned.
End Date displays the date on which the person (or group) becomes (or became) inactive or unassigned.
