Use this tutorial to add an evaluation/assessment authorization for IFSP meetings on the IFSP Meetings page.
Complete the following steps to perform this tutorial:
Display the IFSP Meetings page.

Depending on the method of access and your current permissions, the page displays differently and some information may be disabled or unavailable. |
Double-click on a meeting in the IFSP Meetings table. The page refreshes and displays detailed information below the table.
View the selected meeting date and status in the IFSP Meetings section heading.
Use the function buttons to manage the IFSP meeting:
Click Cancel Meeting to cancel a scheduled IFSP team meeting.
Click Reschedule Meeting to reschedule a scheduled IFSP team meeting.
Click Household Worksheet to display the Household Date Worksheet in PDF format.
Click Print IFSP to display the Individualized Family Service Plan in PDF format for finalized meetings.
Click Finalize IFSP to finalize an on-going IFSP team meeting.
Click Print List to print a list of authorizations in PDF format.
Click Delete Meeting to delete a scheduled IFSP team meeting.
Click Schedule Meeting to schedule a tentative IFSP team meeting.
Click the Authorizations link. The page refreshes and displays an Authorizations table.

NOTE: Depending on the status of the selected meeting, the data displayed on the page may be read-only or unavailable. Meetings with a status of On-going or Scheduled can be updated; however, some information populated from previous meetings is view-only. Meetings with a status of Finalized are read-only and cannot be updated. For meetings with a status of Tentative, Cancelled, or Rescheduled, this page is not available. |
Click the Add button. The page refreshes to display additional fields for entry.
Complete the following information as necessary:
Authorization # displays the identification number for the authorization.
In Authorization Type, select the authorization type from the drop-down list. The page refreshes and different fields are available for entry depending on the type selected.
In Service Type, select the service type from the drop-down list. The page refreshes and different fields are available for entry depending on the type selected.
Select the Compensatory Service check box to indicate whether payment can be rendered for the service.
Authorization Status displays the current status of the authorization.
In Start Date, enter the date on which the activity begins. When updating detailed information, the start date may be disabled. For more information about entering dates, see the Calendar Controls topic.
In End Date, enter the date on which the activity is completed. For more information about entering dates, see the Calendar Controls topic.
Click Cancel Authorization to cancel/discontinue the authorization. A Date field is displayed. Enter the cancellation/discontinuation date for the authorization. For more information about entering dates, see the Calendar Controls topic.
Cancel Date displays the date on which the activity was discontinued or canceled.
Select Evaluation/Assessment in Auth Type and select any option in Service Type. The page refreshes and displays additional fields for entry.
Update the Related Domains section by selecting one or more of the following:
Select the Thinking/Learning check box to indicate that a person's thinking or learning ability is related to the activity.
Select the Communication check box to indicate that a person's communication skills are related to the activity.
Select the Self-Help check box to indicate that a person's ability for self-help is related to the activity.
Select the Movement check box to indicate that a person's mobility is related to the activity.
Select the Social/Behaviors check box to indicate that a person's social skills or behavior is related to the activity.
Select the Vision check box to indicate that a person's vision is related to the activity.
Select the Hearing check box to indicate that a person's hearing is related to the activity.
Update the Evaluation & Assessment Information section as follows:
In Reason, the appropriate radio button is pre-selected based on whether eligibility is being determined or the person is in IFSP.
In Method/Location, select the method and location of the service or meeting from the drop-down list.
EI Code displays the EI identification number and description.
In Frequency, enter the frequency of the activity.
In Per, select from the drop-down list to further define the frequency of the activity.
In Max Miles, enter the maximum number of miles for the transportation authorization. This field is used only for Transportation & Related Cost authorizations (as selected in Service Type).
In Intensity, enter the intensity of the activity.
In Comments, enter additional notes or details. At least 1 character must be entered, and up to 1000 characters can be entered as necessary.
Update the Provider Information section. It contains a radio button group that provides access to various functions on the page. The radio button group consists of the following radio buttons:
The Provider radio button is used to search for and add provider information to the authorization.
The No Provider Available radio button is used to indicate that no provider information is available.
When indicating whether or not provider information is available, the page displays differently and some information may be disabled or unavailable.
If no provider is available, complete the following information:
In Specialty, select the specialty needed for the service from the drop-down list.
In Reason, select the reason a provider was not available to perform the service from the drop-down list.
If provider information is available, click the Provider link to search for and select the specific provider to add. The Provider Search page displays when clicked.
Complete information in one or more of the following fields to perform a search:
In First Name, enter the first name of the person on which to search. You can enter partial information to widen your search. This is an optional field. You can enter up to 25 characters as necessary.
In Last Name, enter the last name of the person on which to search. You can enter partial information to widen your search. At least two characters must be entered, and you can enter up to 40 characters as necessary. The Last Name is required when searching by name.
In ZIP, enter the ZIP code on which to search. You can enter partial information to widen your search.
In Payee, enter the name of the payee on which to search. You can enter partial information to widen your search. At least two characters must be entered.
In Specialty, select the specialty of the person on which to search from the drop-down list.
Click Search. The page refreshes and results found to match the search criteria display in the results table.
When Search is clicked, results found to match the search criteria specified are displayed in the following column(s) of the table:
Last Name displays the person's last name.
First Name displays the person's first name.
Specialty displays the individual's specialty.
County displays the name of the county in which the person resides (or practices).
Phone displays the telephone numbers at which the person (or primary contact) can be contacted.

NOTE: If no results are found, the table displays a single row containing the text: "No Records Found". For more information about tables, see the Tables topic. |
Double-click on a provider in the results table to select the provider. The IFSP Meetings page refreshes and displays the selected provider's details:
Provider Name displays the name of the service provider.
Specialty displays the individual's specialty.
Phone displays the telephone numbers at which the person (or primary contact) can be contacted.
Payee Name displays the name of the person or group to which payment is made.

NOTE: Asterisks (*) indicate which fields are required on the page. If required information is missing, an error message displays at the top of the page after saving the data. |
Click the Save button. The page refreshes and displays the new information in the Authorizations table.
